We are pleased to accept registration requests from patients who are living within the area indicated in the map below.
If you live here, or are moving into this area, you are welcome to apply to register with our practice.
To prevent patient registration fraud within the NHS, we require a combination of the following items of identification to enable us to process your registration:
- birth certificate
- marriage certificate
- medical card
- driving licence
- passport
- local authority rent card
- paid utility bills
- bank/building society cards/statements
- National Insurance number card
- payslip
- letter from Benefits Agency/benefit book/signing on card
- papers from the home office
- P45
How can I register with Langport Surgery?
- Register ONLINE via the NHS App or this link.
- Visit the surgery to collect paper Registration forms.
- Print off, complete and bring your forms into the surgery, Click the blue links below for applicable forms:
Registration will be with the practice and not a specific GP, although you will be allocated to the list of one of the GPs as your usual GP.
You have the right to express a preference to see a particular GP, which will be entered on your record.

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.Named GP
Langport Surgery will ensure that there is a named accountable GP assigned to each patient. New patients will be allocated a GP at the time of registration.
Where patients express a preference for a particular GP the surgery will make every effort to accommodate this request. You may still choose to see any GP of your choice, as you currently do, even if they are not your named GP. If you would like to know who your named GP is please ask a member of our team.
